First American Payment Systems Applications

Merchant Application - Monthly Minimum
Sets the terms and conditions for merchants to accept bankcards with a monthly minimum discount of $25. No annual fee.
   Additional Outlet - Monthly Minimum
    Allows merchants to add locations to accept bankcards with a monthly minimum discount of $25.
Merchant Application - Annual Fee
Sets the terms and conditions for merchants to accept bankcards for a $95 annual fee. No monthly minimum.
    Additional Outlet - Annual Fee

Internet Addendum
This form is an addendum to the merchant application and should be filled out by every merchant applying to accept credit cards online.

LEASE APPLICATIONS

Merimac Lease
Sets the terms and conditions and schedule of payments for leasing point-of-sale equipment through Merimac Capital, a division of First American Payment Systems.

CHECK GUARANTEE APPLICATIONS

Secur Chex Application
Sets the terms and conditions for merchants to guarantee checks through Secur-Chex.

DEBIT & EBT APPLICATIONS

Debit Application
Sets the terms and conditions for merchants to accept debit or Electronic Benefits Transfer (EBT) cards.  To accept EBT, the merchant must also fill out the EBT Services Rider Agreement.
EBT Service Rider  
Sets the conditions of the agreement to accept EBT cards.
EBT Wholesale Rider
The EBT Services Rider for Wholesale Merchants sets the conditions of the agreement to accept EBT cards.

ATM APPLICATIONS

ATM Credit Application
The ATM Credit Application authorizes the release of credit information to First American.
ATM Owner Agreement
The ATM Owner Agreement sets the terms and conditions of the agreement between First American and the ATM owner. It lists information about the ATM owner, location information, equipment information, bank account information and a schedule of charges.
ATM SetUp Agreement
The ATM Set-Up Agreement provides information about the ATM owner, location information, ATM information, and hours of operation.

FIRSTPAY.NET APPLICATIONS

FirstPay.Net License
Sets the fees and the terms and conditions of the license for merchants to use FirstPay.Net.
FirstPay.Net Processing Agreement 
Sets the fees and the terms and conditions for merchants to process transactions using FirstPay.Net.
FirstPay.Net Set Up Form
This form is to be filled out by the web designer detailing the website’s technical information

Please complete, sign and forward the appropriate documents with the items listed below to:

Please include the following items with your application:

     • "Void" Check from Business Checking Account
     • Pictures of Business Location (Inside / Outside)
     • Copy of Business Certificate (if no permanent sign on business location) 
     • Copy of Signer's Driver's License 
     • Copies of last three (3) month's card processing statements (if you accept cards now)
     • Marketing Material, Business Card, Website Address, etc.
     • Application Fee Payable to FAPS
     • First & Last Lease Payment or Purchase Payment payable to FAPS

If you will be processing over $50,000 per month please also include:

     • Most recent financial statements (profit / loss and balance sheet)
     • Copies of last two (2) years tax returns (business and personal) 
     • Copies of last two (2) month's business checking account statements 

Upon receipt we will process your application, and on approval, usually within a few business days, send your terminal, software or Internet gateway set up instructions so you can start accepting credit cards right away... It's that simple!
